Diabetes Regional Coordinating Centre in the Champlain LHIN
As part of the Ontario Diabetes Strategy, the Ministry of Health and Long Term Care is creating a Regional Coordinating Centre (RCC) in each Local Health Integration Network (LHIN). As a MOHLTC initiative, the RCCs are responsible for the coordination of and improvements in outcomes across the continuum of care, from prevention to primary and tertiary care, so that individuals living with, or at risk of developing diabetes have access to the services they need as close to home as possible.
The RCCs will leverage local resources and existing expertise to ensure coordination of diabetes services across the Champlain region. The RCC will demonstrate the region’s ongoing commitment to quality care, clinical excellence and client satisfaction.
This new initiative is aligned with the Champlain LHIN’s mission to link services that help people maximize their health by building a coordinated, integrated and accountable health system for people where and when they need it. The Champlain region covers close to 18,000 sq. km. with a population of 1.2 million. This region encompasses urban and rural differences, language and cultural diversity, including a significant aboriginal and francophone population, as well as economic variability.
Position: Diabetes Outreach Coordinator
Description: The Diabetes Outreach Coordinator works as part of the Diabetes Regional Coordination Centre (DRCC) to achieve the goals and objectives of the DRCC and the Ontario Diabetes Strategy. This position maintains links with diabetes education teams and other groups and organizations in the region to assist with diabetes management.
Reporting Relationships
The Outreach Coordinator is a member of the DRCC and reports to the DRCC Regional Administrator.
Major Responsibilities
- Builds awareness of diabetes services and participates in the dissemination of diabetes standards of practice across the region.
- Works collaboratively with other groups and organizations in the region (primary care physicians, Family Health Teams, Nurse-Practitioner Led Clinics, Champlain LHIN, diabetes education programs and other agencies) to assist with diabetes service coordination.
- Works as part of the Regional Coordination Centre (RCC) team to achieve the goals and objectives of the Diabetes RCC and the Ontario Diabetes Strategy (ODS).
- Assists in the development and implementation of short and long-term strategies to enhance diabetes service provision.
- Assists in the development and delivery of pertinent standardized training to staff new to diabetes education and to more experienced staff who need to advance their practice towards diabetes management.
- Works collaboratively with sub committees to develop and implement common tools and resources to improve diabetes management across Champlain.
- Works collaboratively with the DRCC Team and sub committee to develop newsletters, brochures, fact sheets and publications to distribute to the Diabetes Education Programs and Partners within the Champlain LHIN.
Some travel and regular communication with stakeholders throughout the Champlain LHIN is
required using own vehicle.
The required qualifications of this function are:
Essential
- Bachelor degree in Nursing or Nutrition.
- Five years experience in diabetes education and management.
- Strong organizational skills.
- Strong interpersonal skills.
- Excellent verbal and written communication skills in English and French.
- Proficient with Windows-based computer systems, in particular e-mail and MS Word.
Desirable
- CDE certified
- Experience working with a multi-disciplinary teams
Language designation
English & French, written and spoken
This full-time position is funded through the Diabetes Regional Coordination Centre. Salary is
commensurate with experience, including an excellent benefit package.
