Service Description: Intake and referral program provides easy access to resources for adults living with prediabetes or type 2 diabetes or providing care for those who are * Diabetes education provides support, practical tools and guidance in individual or group sessions * programs are co-facilitated by a Registered Dietitian and a Registered Nurse * seven community diabetes education programs offering programs at 20 locations * a range of services including how to manage diabetes and insulin management * sessions are suitable for both newly-diagnosed patients or refresher courses for patients living with diabetes for years * family members are welcome

Application: People living with diabetes can self-refer with the online form or by calling the office phone * healthcare providers can refer a patient by downloading the form from the website, completing it, and faxing it to 613-233-9487
