Service Description: Foot care * amputation prevention program for people with diabetes having foot ulcers, open wounds and/or ingrown toenail with bacterial infections who are without private insurance

Application: Referral from health care provider required * must use the referral form, email, fax or call to receive a copy * no referrals accepted for routine care
Eligibility - Population(s) Served: People 18 years and older with diabetes having open wounds and/or infections and who are without private insurance * individuals do not have to be patients of the Family Health Team to receive service at this location
