Service Description: Coordinates all admissions into long-term care homes (including short-stay and respite care beds) and into one complex continuing care (chronic care) facility across Champlain. * Refers placement applications and assists in the coordination of admissions of long-term care facilities in other regions of Ontario

Application: Individuals can phone directly or they can be referred by family, friends or another professional.
Eligibility - Population(s) Served: Referrals for placement services are accepted from institutions, agencies, physicians, hospitals, individuals in the community. Professional staff visit, counsel and assess referred persons and determine if they meet the eligibility criteria for placement/admission to a long-term care facility.
