Summer: day trips to local attractions * events

Champlain Going Home Program, Ottawa site: referring patients at time of discharge after hospital stay or release from the Emergency Room * assistance for up to 10 days after referral for:
  • Meals on Wheels
  • transportation home
  • follow up appointments
  • homemaking
  • information on community support services in local community for seniors
