Service Description: Rehabilitation and health care * specialized services for seniors available * services are personalized and flexible from as little as a few hours to 24 hours a day

Life Skills Workers: Work with individuals who have sustained a brain injury * can expedite recovery and equip the client with appropriate physical, cognitive, behavioral and social skills * workers work with mental health clients to build community contacts and independent living skills

Attendant Care Workers: Provide and document care for the client * report observations to other members of the health care team * accompany clients to appointments outside of their home or hospital settings

Home Management: Personal care * meal planning and preparation * laundry and cleaning

Cognitive Rehabilitation: Attention and concentration * memory * safety and judgment * goal setting * organization * problem solving / reasoning * scheduling and time management

Community Reintegration: Educational goals * work and volunteer placements * public safety * banking * shopping, grocery shopping * public transportation * recreation and leisure

Physical Therapy: Mobility * strength training * endurance * exercise programs
