Service Description: Designed to help reduce cardiovascular risk factors. A mentor will provide personalized, confidential and free intervention plan to achieve a better heart healthy lifestyle.

A full evaluation of risk factors is done at the beginning and at the end of the 3 month program during which a mentor will call once a week to monitor and provide information on exercise, nutrition, smoking cessation (if needed) and stress management. A one-year follow-up, including updating the cardiovascular risk profile, is offered to all participants. A copy of the cardiovascular risk profile is also provided to the referring physician at each time point.

Application: Referral form available on the website * medical referral by family physician or nurse practitioner is needed
Eligibility - Population(s) Served: This program is open to Franco-Ontarians 18 years and older who have one or more risk factors for heart disease, or those who have cardiovascular disease
Age: 18.00 years and older
