Service Description: Risk and needs assessments provided by Registered Nurses to assist clients to stay safely in their own homes and maintain their quality of life * holistic care management services to seniors to supplement government services * assistance provided to clients wanting to transition to community living
  • medication management
  • fall prevention
  • professional liaison services: reports to family, physicians and other services as required
  • accompaniment to medical appointments and other health appointments
  • home visits and follow-ups as needed
  • support and guidance to caregivers
  • client advocacy
  • companion service
  • coordination of care and services
  • care and service coordination

Fees: Service - Free phone assessment * $150 in-home assessment * $85 monthly health and safety follow-up visits (2015) * $50 weekly health and safety home visit * $24 companion service & accompaniment to health appointments
Application: Appointment required * no referral needed
Eligibility - Population(s) Served: Individuals 50 years and older * anyone experiencing a health crisis, is recovering from surgery or a fall, dealing with other health issues, needing guidance/support to navigate the health system or to determine what is needed to remain safely at home
Age: 50.00 years and older
