Service Description: Support for persons with dual diagnosis in the mental health and developmental service sectors

Provides direct services to people who have an intellectual disability and a mental illness * Services are time limited (up to 1 year) and include weekly mental health case management, advocacy for individuals by linking them to appropriate community services and supports * increases access to appropriate services and supports * facilitates training to developmental and mental health services professionals to increase awareness of specific needs of clients and their families * facilitates short and long-term planning to clients and their support networks * monitors and assesses service system gaps and reports findings to relevant ministries *  works with community partners in the developmental services sector as well as the mental health sector

Application: Individuals eligible for the Developmental Services Ontario Eastern Region (DSOER) must be referred by the DSOER
Eligibility / Target Population: Persons 18 years and older with a dual diagnosis and complex needs (developmental disability and mental health diagnosis)
