Service Description: A partnership of 6 Ottawa-based agencies (MHCSS) that provide individualized intensive case management support to persons experiencing severe and persistent mental illness and/or co-occurring substance use, who require support in order to live in the community. Acts as a centralized access point for intensive case management services which is coordinated by CMHA Ottawa

Intensive Case Management: A community based mental health service. Services include: outreach * assessment and planning * direct service provision * monitoring and follow up * information * consultation * and advocacy. The service model draws from strength-based case management, psychosocial rehabilitation and recovery. Individuals receiving intensive case management may also be able to access other integrated services and treatment groups, including Concurrent Disorders Treatment groups and Smoking Cessation

Fees: None
Application: Self-referral * referral by family members, or professionals * person being referred must sign the consent in the application package, consenting to and acknowledging the referral
Eligibility / Target Population: Adults 16 years and older with a severe and persistent mental health problem who need significant support in order to access services that are needed and wanted
