Service Description: Management of acute and chronic foot conditions including assessment and treatment * nail surgeries for ingrown toenails * evaluation of current footwear * advice on proper footwear * Total Contact Casting (TCC) if appropriate

Application: Referral from health care provider required * must use the Champlain Diabetes Chiropody Program referral form and criteria available on the website * No referrals accepted for routine care
Eligibility / Target Population: People 18 years and older with diabetes having open wounds and/or infections and who are without private insurance * individuals do not have to be patients of the Family Health Team to receive service at this location
