Service Description: Falls prevention program * works with seniors, health service and community agencies to reduce the number of falls and their impact on seniors, their caregivers and the health care system

Assessment and intervention for high-risk seniors on an individual basis * education, information and health promotion for seniors and caregivers on fall prevention * advocacy and identification of gaps in services for seniors

Application: Referrals are accepted from any source, call to obtain referral form or to complete intake over the phone * The program services the West end region of the City of Ottawa - to check catchment eligibility, call the program
Eligibility / Target Population: Seniors 70 years and older in the West End of the Ottawa Region, who have had 1 or more indoor and/or outdoor falls in the last year, who live in their own home or retirement home, who are not currently receiving services through Ontario Health atHome or a day hospital, and are willing and able to participate in taking action to reduce their risks of falls
