Service Description:

Intensive treatment services in mental health in clients' home

assistance in finding suitable housing
medication management
help to reduce harm caused by addictions
promotion of community integration
help to find other health care services such as a physician or therapist
peer support
development of personal interests and leisure activities

Self-referrals accepted * physicians, health care professionals, and community organizations that dispense health care services may also refer clients * for all new referrals, call central intake, toll free 1-844-304-1414

Eligibility / Target Population:

Individuals 16 years and older with schizophrenia, bipolar disorder, or other severe, persistent, and/or recurrent mental health problems
