Position:  Bilingual Intake / Case Manager Social Services

Region:  Ottawa

Description:  Bilingual Intake / Case Manager Social Services (General Counselling, 2SLGBTQ+ Counselling and Trans Health Counselling), Manager Mental Health Substance Use Health and Addictions (MHSUHA)

Summary
The Counselling programs offer services for 2SLGBTQ+ youth (16-25), Trans health counselling for youth and adults as well as General Counselling for youth and adults. The Social Services Counselling program Intake/Case manager is often the first point of access to individuals into the Counselling program. The incumbent will follow up with self-referrals and professional based referrals and assess eligibility for counselling. They will do an intake and assessment that captures the current issues experienced by the individual and explore the hopes and goals for making changes. As part of the assessment process the need for case management/care coordination support to vulnerable clients who may need assistance with one or more areas of their life (ie. Housing support/ health system navigation etc.) will be determined at that time.
The Social Services Counselling intake/case manager will be responsible for supporting clients on the waitlist when applicable, through the provision of individual support, case management supports or through the provision of psycho-educational groups as needed.

Strong assessment skills in mental health and addictions, along with an understanding of best practices in integrated care for youth and adults and more specifically with 2SLGBTQIA+ individuals including Trans or Gender Diverse individuals, is required for this position. Also important is the ability to inform and refer clients to resources when clients do not meet the eligibility criteria for our program. System navigation experience is required.

This position will also work with the LESA intake worker/case manager and provide coverage at times.

Qualifications:
Essential

  • Bachelor’s degree in Social Work, Nursing, Health Science, or equivalent combination of education, training and experience will be considered
  • Lived Experience as a member of the 2SLGBTQIA+ community (please self-identify)
  • 3 years’ mental health and addictions intake and case management experience
  • Experience and knowledge related to working with youth and adults
  • Proven ability to work well independently and as part of multidisciplinary team
  • Knowledge of a Harm Reduction Approach
  • Ability to provide case management services including within a concurrent disorders framework
  • Knowledge of recovery principles within mental health
  • Demonstrated ability to provide crisis intervention and support
  • Understanding of the importance of community-based care designed to support youth and adults in keeping with CCHC’s commitment to equity, anti-racism, and anti-oppression.
  • Demonstrated knowledge of working with equity deserving youth and adult populations including: indigenous individuals, 2SLGBTQIA+ individuals, Trans and Gender Diverse individuals, those with concurrent disorders; racialized persons/people and older adults on low income.
  • Demonstrated ability to work with vulnerable populations within a trauma-informed approach
  • Demonstrated understanding of and experience with Motivational Interviewing
  • Ability to inform and refer clients to community resources when they are not meeting the criteria for the program (excellent knowledge of community resources and system navigation)
  • Proven ability to network and build professional relationships
  • Strong organization and time management skills
  • Excellent verbal and written communication skills
  • Flexibility, excellent judgement and initiative
  • Ability to work some evenings and weekends
  • Proficient with Windows-based computer systems including office 365 email, MS Word and data base and EMR programs
Desirable
  • Experience in-group counselling.
  • Lived experience as a Trans or Gender Diverse individual (please self-identify)

Salary:  $ 53,421 - $ 64,362 / annual

Terms of Employment:  Temporary Part-Time, 6 months
