Diabetes Education Programs - North Lanark/North Grenville

Diabetes education programs provide individuals with the tools, skills and confidence needed to properly self-manage their diabetes and enable them to live healthy lives. The diabetes education teams consist primarily of a registered nurse and a dietitian who work with family physicians and diabetes care experts. People with diabetes are encouraged to take an active role in the day-to-day management of their own health care and can access one of Ontario's Diabetes Education Programs for assistance.

The Métis Nation of Ontario, Diabetes Education Project provides foot care funding.

For chronic disease self-management programs, visit Living Healthy Champlain.
