Service Description:
|
One-on-one nutritional consultations for clients aged 18 years of age or older living with a new or chronic condition eg weight management, malnutrition, irritable bowel syndrome, renal disease, diabetes
Services include OTN diabetes clinics with an endocrinologist, and in person diabetes care clinic joint visits with CPDMH dietitian and RCHS nurse
