Service Description:
|
Companionship, respite care, and emotional support
Residents:
- physical, emotional and spiritual support
- focus on pain and symptom management
Home Support Program:- initial visit by a community nurse for assessment and referral
- volunteer visiting for companionship and practical support
- respite for families up to four hours per week
- telephone support
- caregiver support
Day Hospice Program: - weekly social interaction with peers
- variety of activities including art, cards, board games, music
- emotional support
- respite for caregivers
- weekly nurse assessments and communication with physicians as needed
Bereavement Support: Support through the grieving process by staff and trained volunteers
