Service Description:

Access point for prediabetes and type 2 diabetes community education * intake and referral * individual and group sessions offered by registered dietitians and registered nurses * services include lifestyle management, insulin initiation and medication/insulin adjustments * education sessions are suitable for both newly-diagnosed adults and refresher courses for adults living with diabetes for years; family members are welcome

People living with diabetes can self-refer via the online self-referral form on the website or by calling * healthcare providers can refer patients by downloading the referral form from the website and sending it by fax

Adults 18 years and older living with type 2 and prediabetes
