Service Description: Supports adults with developmental disabilities to live full, meaningful lives in their communities by helping them make choices about their lives, form lasting relationships, access opportunities, and experience a true sense of belonging

Community Participation Support (CPS - Day Program): Program activities focus on academics, technology and computer skills, music, games, nutrition and wellness, safety awareness and community involvement * Participants work alongside peers to build skills, confidence and independence * Funded spaces are filled through DSOER referrals and waitlists * CPS services may also be accessed using Passport funding or a fee-for-service arrangement with OCL, based on availability

Host Family/Home Share Program: provides adults with developmental disabilities the opportunity to live long-term in a Home Share provider's home * Providers offer support in all aspects of daily living, including routines, appointments, community participation and inclusion in family activities

Supported Group Living Homes: Provides 24/7 staffing and individualized support for adults who require ongoing assistance with daily living, including those with complex care or high medical needs * Support is tailored to each person's needs and goals and may include skill development, behavioral supports, increased independence and community participation * All vacancies are filled through DSOER referrals and waitlists

Supported Apartment Living: Individuals share an apartment with one other person and receive 24-hour staff support, seven days a week * A small number of one- and two-bedroom apartments are located close together, with technology used to support staff in monitoring and responding to individual needs * All vacancies are filled through DSOER referrals and waitlists

Supported Independent Living (SIL Program): provides supports to adults who want to build the skills and confidence needed to live more independently * Support is tailored to each person's goals, strengths and needs with a focus on daily living, community participation and personal growth * supports can be accessed through a DSOER referral and waitlist, Passport funding, or a fee-for-service arrangement with OCL, based on availability

Moving On Out (MOO Program): a 4-6 week, hands-on learning program that helps individuals build the practical skills and confidence needed to live more independently * Held in a fully furnished community apartment with a maximum of two participants and one Direct Support Professional, the program provides an individualized, realistic and supportive environment for developing daily living skills and routines * Funded spaces are filled through DSOER referrals and waitlists
