Service Description:
|
Non-medical assistance with activities of daily living in the home, such as dressing, bathing, eating, and helping with range of motion * Attendants provide physical assistance with activities of daily living such as personal grooming, transferring, dressing, bathing, meals, ventilator, tracheotomy services, and more
Supportive Housing program is a shared scheduled services model, with on-call services available when needed * clients reside in their own units within Supportive Housing Program; these units are provided through the Ottawa Social Housing Registry as Rent-Geared-to-Income (RGI) * An additional application for RGI will have to be completed with the Registry
Outreach Attendant Services available within the Pembroke-Renfrew county area * Fee-for-service also available
Applicant must:
- Be 16 years of age, or older
- Have Provincial health coverage and a valid health card
- Have a permanent physical disability or functional limitation which results in the inability to independently complete any of the following examples of personal care routines: e.g., toileting/bowel routines, hygiene, washing/bathing, dressing, transfers/positioning, eating, medication, tracheostomy care
- Require assistance with personal care from MODC as a result of this physical disability/ functional limitation
- Be able to self-direct care i.e., the individual is capable of managing, determining, and communicating when/how assistance is needed
- Have a service location that has/will have the physical features necessary for MODC to deliver its services without a significant risk of serious physical harm to staff
- In the event there is a significant risk of serious physical harm to staff providing services, to: collaborate with MODC to help it identify reasonable steps that MODC can take in reducing the risk to a level that is no longer significant
- Not be in receipt of, or expecting receipt of, insurance funds for provision of attendant services
- Any other funder-specific eligibility criteria as applicable (e.g. geographical area, age, etc…)
- Be unable to have their needs met effectively in the community on a scheduled/unscheduled basis (i.e., through MODC outreach or other existing community resources); and require availability of/access to 24/7 staffing
- Have support needs, including existing medical condition(s) that can be adequately met by MODC onsite staff within available hours or by local community health services; and
- Be able to be safely left unattended within a community-based shared service model
